From referral to routine.

Below is the step-by-step referral process we will follow once vendorized, from initiation by a Service Coordinator through the start of services. Specific details may vary by regional center.

The referral process

From initiation by a Regional Center Coordinator through the start of services. Each step builds on the one before it.

  1. Referral Initiation

    Referrals are initiated by a regional center Service Coordinator through the Individual Program Plan (IPP) process.

  2. Referral Submission

    The regional center provides the vendor with a referral packet, which includes the current IPP and relevant behavioral, medical, and safety information.

  3. Referral Review

    The vendor reviews the referral to determine program compatibility, capacity, staffing availability, and ability to meet the individual's behavioral support needs.

  4. Intake

    If appropriate, the vendor completes an intake via Telehealth or in person, as needed, to gather background information and confirm alignment with the individual's IPP goals.

  5. Assessment

    An in-person observation, including interviews with caregivers and staff, a review of records, and the completion of one or more standardized assessment tools such as the FAST, Vineland-3, ABLLS, AFLS, or Socially Savvy, to understand support needs.

  6. Admission Decision

    Via Home and Community Services notifies the regional center Service Coordinator of acceptance or denial. Any denials will include written justification.

  7. Authorization

    Services begin only after written authorization is received from the regional center for the Behavioral Day Program.

  8. Service Start

    Once authorized, the individual begins participation in the Behavioral Day Program in alignment with the IPP and regional center requirements.
